Abstract

Traditional disease-centric approaches often fall short in addressing the dynamic and interrelated nature of multimorbidity. This review synthesizes current clinical guidelines for adaptive multimorbidity care, emphasizing the use of dynamic health indicators to guide management. Integrating these indicators into clinical decision-making enables adaptive, patient-centered care pathways that are responsive to fluctuating needs, disease interactions, and treatment burdens.
