Otorhinolaryngology (ENT) encompasses a diverse range of disorders affecting the ear, nose, and throat, presenting significant diagnostic and therapeutic challenges. Advances in surgical and non-surgical techniques over the last decade have markedly influenced patient outcomes, with a shift toward minimally invasive approaches, precision diagnostics, and multidisciplinary care. This article aims to synthesize current evidence, clinical guidelines, and expert perspectives to inform best practices in ENT, emphasizing practical implications for patient management and outcome optimization.
ENT disorders constitute a substantial global health burden, with conditions such as chronic rhinosinusitis, otitis media, and head and neck cancers ranking among the most prevalent. The World Health Organization estimates that over 5% of the world population experiences disabling hearing loss, while allergic rhinitis affects up to 30% of adults. In pediatric populations, otitis media remains a leading cause of healthcare visits and antibiotic prescriptions. The morbidity associated with upper airway diseases, sleep-disordered breathing, and neoplasms further underscores the need for effective, evidence-based interventions.
ENT diseases arise from a complex interplay of infectious, inflammatory, neoplastic, and structural mechanisms. For example, chronic rhinosinusitis is characterized by persistent mucosal inflammation and impaired mucociliary clearance, while otosclerosis involves abnormal bone remodeling within the otic capsule. Head and neck malignancies often develop from genetic mutations and environmental exposures, leading to dysregulated cell proliferation. Diagnostic strategies in ENT have evolved with advancements in imaging, laboratory testing, and endoscopic visualization. High-resolution computed tomography (CT) and magnetic resonance imaging (MRI) are pivotal in assessing sinonasal and temporal bone pathology. Nasal endoscopy, fiberoptic laryngoscopy, and otoacoustic emissions provide direct visualization or functional assessment. Molecular diagnostics and immunohistochemistry now play a central role in tumor subtyping and prognostication. Accurate diagnosis depends on integrating clinical, radiologic, and pathologic information, guided by current best practice standards.
Management of ENT disorders involves a spectrum of medical and surgical interventions, customized to disease type, severity, and patient comorbidities. Medical therapies include intranasal corticosteroids for rhinosinusitis, antibiotics for bacterial otitis media, and antihistamines for allergic conditions. Surgical techniques have transitioned toward minimally invasive paradigms, such as endoscopic sinus surgery, cochlear implantation for sensorineural hearing loss, and transoral robotic surgery for selected tumors. Multidisciplinary collaboration is increasingly recognized as key to optimizing outcomes, especially in complex or refractory cases.
The field of ENT is witnessing rapid innovation. Balloon sinuplasty offers a less invasive alternative for selected chronic sinusitis cases, while image-guided surgery has improved precision and reduced complication rates. Immunotherapy and targeted biologics, such as anti-IL-5 agents for nasal polyposis, represent a paradigm shift in managing refractory inflammatory diseases. In hearing restoration, advancements in bone-anchored hearing devices and gene therapy research hold promise for previously untreatable conditions. The integration of artificial intelligence in diagnostic imaging and intraoperative navigation is poised to further enhance safety, efficacy, and personalized care.
Recent clinical guidelines from bodies such as the American Academy of Otolaryngology–Head and Neck Surgery (AAO-HNS) and the European Position Paper on Rhinosinusitis and Nasal Polyps (EPOS) emphasize evidence-based decision-making.
