Functional goal setting is a cornerstone of effective rehabilitation, aligning patient aspirations with clinical objectives to drive meaningful recovery. Traditionally, rehabilitation goals were set through multidisciplinary collaboration, but emerging evidence underscores the unique contributions of nurses in leading this process. Nurse-led functional goal setting leverages the nurse’s holistic patient assessment skills, advocacy role, and frequent patient contact to tailor interventions and enhance engagement. In high-income countries, up to 40% of hospital discharges involve patients needing post-acute rehabilitation. The increasing prevalence of chronic diseases and aging populations further amplifies the demand for effective, patient-centered rehabilitation strategies, highlighting the critical need for structured and individualized goal-setting processes.
The need for rehabilitation arises from a myriad of pathophysiological processes leading to functional decline.
