Introduction

Chronic cough, persisting for more than eight weeks, poses a significant diagnostic and therapeutic challenge in clinical practice. This article aims to provide a comprehensive approach to managing this common yet complex condition.

Etiology and Pathophysiology

Chronic cough is often a symptom of underlying conditions such as gastroesophageal reflux disease (GERD), asthma, and upper airway cough syndrome (UACS). It could also be idiopathic in nature. The pathophysiology involves a hypersensitive cough reflex triggered by inflammation and neuronal changes.

Diagnostic Approach

Initial evaluation should aim to identify common causes using a thorough history, physical examination, and basic investigations such as chest X-ray and spirometry. If the cause remains elusive, further investigations including high-resolution computed tomography, bronchoscopy, and esophageal pH monitoring may be warranted.

Treatment Strategies

Treatment should be tailored to the underlying cause. For GERD-induced cough, proton pump inhibitors are recommended. Inhaled corticosteroids are beneficial in asthma-related cough, while antihistamines and decongestants may help in UACS. If the cough is idiopathic, neuromodulators such as gabapentin can be considered.

Refractory Cases

In refractory cases, a multi-disciplinary approach involving pulmonologists, gastroenterologists, and otolaryngologists may be necessary. Cognitive-behavioral therapy has also shown promise in managing chronic refractory cough.

Chronic cough management requires a systematic and comprehensive approach. Identifying the underlying cause is crucial to guide treatment. In refractory cases, a multi-disciplinary approach and novel therapies may be required. Continued research is needed to better understand the pathophysiology and to develop more effective treatment strategies.
