Abstract

Complex twin pregnancies present considerable challenges for clinicians, requiring nuanced, evidence-based approaches for optimal maternal and neonatal outcomes. This review synthesizes current literature and clinical experiences, focusing on epidemiology, pathophysiology, risk factors, clinical manifestations, diagnostic strategies, management protocols, recent advances, and guideline recommendations. Through a case-based lens, we highlight practical implications and mechanism-driven therapeutic strategies, drawing on recent research and consensus guidelines to inform clinical decision-making in the management of complex twin pregnancies.

Introduction

Twin pregnancies, while accounting for a minority of all gestations, are associated with a disproportionate risk of morbidity and mortality for both mothers and fetuses. Complexity arises from factors such as chorionicity, amnionicity, discordant growth, twin-to-twin transfusion syndrome (TTTS), and preterm delivery risk. Modern perinatal care demands a multidisciplinary, evidence-based approach, integrating advances in fetal imaging, genetics, and intervention. This review aims to provide clinicians with an up-to-date synthesis of the management strategies for complex twin pregnancies, emphasizing case-based learning and practical clinical insights.

Epidemiology / Disease Burden

The incidence of twin pregnancies has increased worldwide, largely due to assisted reproductive technologies (ART) and delayed childbearing. Globally, the twinning rate is estimated at 12–16 per 1000 births, with higher rates in regions utilizing ART. Twin pregnancies contribute significantly to perinatal morbidity and mortality, accounting for 10–20% of perinatal deaths despite constituting less than 3% of all pregnancies. Complications, including preterm birth, growth restriction, and hypertensive disorders, are two to three times more common in twin compared to singleton gestations, underscoring the need for vigilant surveillance and tailored management.

Pathophysiology

The complexity of twin pregnancies is rooted in their unique pathophysiology. Chorionicity monochorionic versus dichorionic fundamentally determines risk profiles. Monochorionic twins, sharing a placenta, are predisposed to vascular anastomoses, leading to complications such as TTTS, selective intrauterine growth restriction (sIUGR), and twin anemia-polycythemia sequence (TAPS). In contrast, dichorionic twins have separate placentas, and while at risk for preeclampsia and discordant growth, are less prone to inter-twin transfusion syndromes. The interplay of placental function, genetic factors, and uteroplacental perfusion underpins the diverse clinical manifestations observed in complex twin pregnancies.

Risk Factors

Key risk factors for complex twin pregnancies include maternal age over 35, ART use, family history of twinning, and ethnicity. Additional contributors to complexity encompass monochorionicity, prior obstetric complications, and underlying maternal comorbidities such as hypertension or diabetes. ART increases the likelihood of monozygotic twinning, which carries a higher risk of complications. Awareness of these risk factors aids in early stratification and anticipatory guidance for affected individuals.

Clinical Features

Complex twin pregnancies may present with a spectrum of clinical features, ranging from asymptomatic to severe maternal or fetal compromise. Common manifestations include disproportionate fundal height for gestational age, polyhydramnios or oligohydramnios, and signs of preterm labor. TTTS typically presents with rapid uterine enlargement, abdominal discomfort, and signs of heart failure in the recipient twin. sIUGR is characterized by discordant fetal growth, often detected through serial ultrasonography. Early recognition of these features is crucial for timely intervention and optimizing perinatal outcomes.

Diagnosis

Diagnosis of complex twin pregnancy challenges relies heavily on advanced imaging modalities. First-trimester ultrasonography is essential for determining chorionicity and amnionicity, which guide subsequent management. Serial growth scans, Doppler velocimetry, and amniotic fluid assessments are integral for monitoring fetal well-being and detecting complications such as TTTS, sIUGR, or TAPS. MRI may be employed for detailed fetal anatomical assessment or when ultrasound findings are equivocal. Noninvasive prenatal testing (NIPT) and invasive diagnostic procedures, such as amniocentesis or chorionic villus sampling, may be indicated in select cases for genetic evaluation.

Treatment & Management

The management of complex twin pregnancies is multidisciplinary, individualized, and guided by chorionicity, gestational age, and specific complications. For monochorionic twins, intensive surveillance is recommended, with biweekly ultrasounds starting at 16 weeks. TTTS may necessitate fetoscopic laser ablation of placental anastomoses, the gold standard for stage II or higher. sIUGR management includes close monitoring and, in selected cases, fetal intervention or early delivery. Preterm labor is managed with corticosteroids, tocolysis, and magnesium sulfate for neuroprotection when appropriate. Maternal complications, such as preeclampsia, require prompt recognition and management according to established protocols. Delivery planning is critical, with mode and timing dictated by fetal status, gestational age, and maternal condition.

Recent Advances / Emerging Therapies

Recent advances in the management of complex twin pregnancies include innovations in fetal surgery, such as minimally invasive fetoscopic techniques for TTTS and selective reduction procedures. Improved imaging technologies, including 3D ultrasonography and fetal MRI, enhance early detection of complications. Emerging research into the molecular basis of placental dysfunction may yield novel biomarkers for earlier risk stratification. Pharmacological interventions, such as antiplatelet agents for preeclampsia prevention and novel neuroprotective agents, are under investigation. Telemedicine and remote monitoring are increasingly utilized for surveillance, particularly in resource-limited settings.

Guideline Recommendations

Consensus guidelines from major societies, including ACOG, SMFM, and RCOG, emphasize the importance of early determination of chorionicity and amnionicity, frequent surveillance, and timely intervention for complications. Biweekly ultrasound for monochorionic twins, prompt referral to tertiary centers for TTTS or sIUGR, and individualized delivery planning are standard recommendations. Antenatal corticosteroids are advised for threatened preterm birth, and magnesium sulfate for neuroprotection before 32 weeks. Multidisciplinary team involvement, including maternal-fetal medicine specialists, neonatologists, and pediatric surgeons, is strongly recommended for complex cases.

The management of complex twin pregnancies remains a dynamic and evolving field, demanding a thorough understanding of pathophysiology, risk stratification, and evidence-based interventions. Early identification of high-risk features, adherence to guideline-directed surveillance, and adoption of emerging therapies are essential for optimizing maternal and neonatal outcomes. Ongoing research and multidisciplinary collaboration will continue to shape best practices, ensuring that clinicians are equipped to navigate the unique challenges inherent in complex twin gestations.
