A thorough history is crucial in the evaluation process. The onset, location, duration, character, and radiation of pain should be noted. Associated symptoms, such as nausea, vomiting, changes in bowel habits, and weight loss, should also be explored. Consideration of the patient's past medical history, medication use, and family history can provide valuable clues.
A systematic physical examination can help localize the pain source. Inspection, palpation, percussion, and auscultation should be performed in all four quadrants. Special attention should be given to signs of peritonitis, hernias, masses, or organomegaly. It's also important to evaluate the patient's general condition, including vital signs and signs of systemic illness.
Investigations should be guided by clinical suspicion. Basic tests include complete blood count, liver function tests, amylase, lipase, and urinalysis. Imaging studies, such as ultrasound or computed tomography, can be useful in visualizing organ pathology. In some cases, endoscopic procedures may be required to further evaluate the gastrointestinal tract.
Management of abdominal pain is condition-specific and may range from conservative measures to surgical intervention. Analgesia should be provided, but with caution to not mask the physical findings. Referral to a specialist should be considered for complex or unclear cases.
Evaluating abdominal pain requires a comprehensive approach, incorporating meticulous history taking, thorough physical examination, and appropriate diagnostic investigations. Understanding the diverse pathologies that can manifest as abdominal pain will aid in accurate diagnosis and effective management, ultimately improving patient outcomes.
