AMS programs aim to optimize antimicrobial use, improving patient outcomes while minimizing unintended consequences of antimicrobial use, including toxicity and the selection of pathogenic organisms.
Developing an effective AMS program requires a multidisciplinary team, including an infectious disease physician, a clinical pharmacist with infectious disease training, clinical microbiologists, information system specialists, and infection control professionals. The team is responsible for implementing interventions, monitoring antimicrobial use and resistance patterns, and providing feedback to healthcare providers.
Key strategies include prospective audit with intervention and feedback, formulary restriction and preauthorization, education, guidelines and clinical pathways, antimicrobial cycling, and combination therapy. These strategies should be tailored to the specific needs and resources of the hospital.
Monitoring and evaluation are critical to ensure the effectiveness of the AMS program. Key metrics include antimicrobial consumption, resistance patterns, patient outcomes, adverse drug events, and cost. Regular feedback to healthcare providers is essential to drive continuous improvement.
Challenges in implementing AMS include lack of resources and expertise, resistance from healthcare providers, and the complexity of measuring outcomes. These challenges can be addressed through adequate training, engagement of hospital leadership, and use of technology to facilitate data collection and analysis.
Implementing effective antimicrobial stewardship in hospitals is crucial to combat antimicrobial resistance. It requires a multidisciplinary approach, tailored strategies, and continuous monitoring and feedback. Overcoming implementation challenges can lead to improved patient outcomes, reduced antimicrobial resistance, and cost savings.
