Assisted reproductive technologies (ART) have revolutionized the landscape of infertility treatment, offering hope to millions of couples globally. The management of patients across the assisted reproductive care continuum requires a multidisciplinary approach, integrating clinical assessment, individualized interventions, and evidence-based protocols to optimize outcomes. This review delineates the essential aspects of patient management from evaluation and diagnosis through treatment and follow-up, highlighting recent advances and guideline-based recommendations. Emphasis is placed on epidemiological trends, underlying pathophysiological mechanisms, risk factor assessment, diagnostic strategies, therapeutic modalities, and future directions in ART. The objective is to provide clinicians and healthcare professionals with a comprehensive, up-to-date resource to enhance patient-centered care in reproductive medicine.
Infertility presents a significant clinical and psychosocial challenge, affecting an estimated 8–12% of couples worldwide. The evolution of ART has expanded the therapeutic armamentarium, enabling successful conception even in cases of complex infertility. Patient management across the reproductive care continuum is inherently multifaceted, necessitating a nuanced understanding of the interplay between biological, psychological, and social determinants. This review synthesizes current evidence and practice guidelines to inform clinical decision-making and optimize patient outcomes throughout the ART journey.
The global burden of infertility is considerable, with approximately 48 million couples and 186 million individuals affected. Primary infertility is more prevalent in low-resource settings, while secondary infertility is often observed in areas with higher maternal age and increased comorbidities. The demand for ART is rising, particularly in developed nations where delayed childbearing and lifestyle factors contribute to declining fertility rates. Epidemiological data underscore the need for accessible, equitable reproductive care and robust public health strategies to address both prevention and management.
Infertility arises from multifactorial etiologies, encompassing female, male, and combined factors. Female infertility may result from ovulatory dysfunction, tubal pathology, endometriosis, or diminished ovarian reserve. Male infertility is commonly attributed to spermatogenic defects, endocrine disorders, or obstructive lesions. ART interventions, such as in vitro fertilization (IVF), intracytoplasmic sperm injection (ICSI), and preimplantation genetic testing (PGT), are designed to circumvent these pathophysiological barriers, facilitating fertilization and implantation.
Risk factors for infertility and suboptimal ART outcomes include advanced maternal age, obesity, smoking, alcohol use, environmental toxins, and pre-existing medical conditions (e.g., polycystic ovary syndrome, endometriosis, diabetes). Genetic predispositions, sexually transmitted infections, and prior pelvic surgeries further compound risk. Comprehensive risk assessment is critical for early identification and tailored intervention, enhancing prognostication and treatment efficacy.
The clinical presentation of infertility is often insidious, typically manifesting as an inability to conceive after 12 months of unprotected intercourse. Associated symptoms may include menstrual irregularities, pelvic pain, hirsutism, or erectile dysfunction, depending on the underlying etiology. Eliciting a detailed reproductive and medical history, together with focused physical examination, is foundational to the evaluation process.
Diagnostic evaluation in ART commences with baseline investigations such as semen analysis, ovarian reserve testing (AMH, FSH, AFC), and assessment of tubal patency (HSG, laparoscopy). Advanced diagnostics, including genetic screening, endocrine profiling, and uterine cavity evaluation (hysteroscopy, sonohysterography), allow for precise etiological characterization. The integration of individualized diagnostic algorithms, informed by clinical presentation and risk stratification, underpins optimal patient selection and workflow efficiency.
Management across the ART continuum is personalized, encompassing lifestyle modification, pharmacologic interventions (ovulation induction, hormonal therapy), and procedural therapies (IUI, IVF, ICSI). Adjunctive strategies include luteal phase support, endometrial receptivity optimization, and management of coexisting comorbidities. Multidisciplinary care, involving reproductive endocrinologists, embryologists, nurses, and mental health professionals, is essential for comprehensive support and improved patient experience. Ongoing counseling and psychosocial interventions are integral to addressing the emotional and ethical complexities inherent to ART.
Recent innovations in ART have focused on enhancing embryo selection, maximizing implantation rates, and minimizing complications. Technologies such as time-lapse embryo imaging, artificial intelligence-driven selection algorithms, and non-invasive preimplantation genetic testing are transforming clinical practice. The advent of personalized ovarian stimulation protocols, improved cryopreservation techniques (vitrification), and advances in reproductive genetics offer new opportunities for individualized care. Ongoing research into uterine microbiome modulation, stem cell therapies, and novel pharmacologic agents holds promise for further improving ART outcomes.
Contemporary guidelines from the American Society for Reproductive Medicine (ASRM), European Society of Human Reproduction and Embryology (ESHRE), and World Health Organization (WHO) emphasize evidence-based protocols for patient assessment, laboratory procedures, and embryo transfer strategies. Key recommendations include individualized ovarian stimulation, single embryo transfer to reduce multiple pregnancy risk, and comprehensive infectious disease screening. Regular updates to clinical practice guidelines ensure alignment with emerging evidence, promoting safe and effective ART delivery.
The management of patients across the assisted reproductive care continuum is an evolving discipline, driven by advances in scientific understanding, clinical practice, and technology. Optimal outcomes are achieved through personalized, multidisciplinary care, adherence to guideline-based protocols, and continuous integration of emerging evidence. Future directions in ART will likely be shaped by innovations in genomics, artificial intelligence, and regenerative medicine, offering new hope for individuals and couples affected by infertility. Ongoing commitment to research, education, and patient-centered care remains paramount in advancing the field of reproductive medicine.
