Vertigo, a subtype of dizziness characterized by a sensation of spinning or feeling off-balance, is a common clinical complaint encountered by healthcare professionals. Despite its prevalence, its diagnosis and management can be complex due to the myriad of underlying causes. This article aims to provide a comprehensive clinical approach to vertigo.
Vertigo is primarily a symptom rather than a diagnosis. It is essential to differentiate it from other forms of dizziness such as presyncope, disequilibrium, and non-specific dizziness. A detailed patient history and a thorough neurological examination can help distinguish vertigo from these other symptoms.
Vertigo can be peripheral or central in origin. Peripheral vertigo is commonly associated with disorders affecting the inner ear or vestibular nerve, such as benign paroxysmal positional vertigo (BPPV), Meniere's disease, and vestibular neuritis. Central vertigo arises from disorders affecting the brainstem or cerebellum, such as stroke or multiple sclerosis. An understanding of these pathologies can help guide diagnosis and treatment.
Diagnosis primarily relies on the patient's history and physical examination. Specific maneuvers like the Dix-Hallpike test for BPPV or the HINTS (Head Impulse, Nystagmus, Test of Skew) exam for stroke can be useful. Further investigations may include audiometry, vestibular function tests, or neuroimaging depending on the suspected underlying cause.
Treatment of vertigo is directed towards the underlying cause. This can range from physical maneuvers for BPPV, medication for Meniere's disease, or more complex interventions for central causes. Symptomatic treatment with vestibular suppressants can be used short term, but long-term use should be avoided due to potential side effects.
Vertigo presents a diagnostic challenge due to its diverse etiologies. A systematic approach to history taking, examination, and investigation can aid in accurate diagnosis and appropriate management. Continued professional development in this area will undoubtedly enhance patient outcomes.
