Acute chest pain, a common yet perplexing clinical presentation, often poses diagnostic challenges for healthcare professionals. The differential diagnosis is broad, ranging from benign musculoskeletal causes to life-threatening conditions such as myocardial infarction. This article provides a comprehensive approach to evaluating and managing patients presenting with acute chest pain.
Initial evaluation should involve a detailed history and physical examination. Key elements include the character, onset, location, duration, associated symptoms, and any exacerbating or relieving factors. Concurrently, vital signs and electrocardiogram (ECG) should be obtained promptly to identify any immediate life-threatening conditions.
Further diagnostic evaluation depends on the initial clinical assessment. For patients with suspected cardiac chest pain, serial cardiac biomarkers and a stress test may be indicated. In cases of suspected pulmonary embolism or aortic dissection, computed tomography angiography (CTA) may be required. For patients with a low pretest probability of cardiac etiology, alternative diagnoses such as gastroesophageal reflux disease or musculoskeletal pain should be considered.
Management depends on the underlying cause of chest pain. Patients with acute coronary syndrome require prompt revascularization, while those with pulmonary embolism may require anticoagulation. It is also essential to address modifiable risk factors and provide appropriate counseling to patients.
Acute chest pain is a complex clinical entity that requires a systematic approach for effective evaluation and management. By tailoring the diagnostic workup based on initial assessment and considering a broad differential diagnosis, healthcare professionals can ensure optimal patient outcomes. Continued education and clinical vigilance are key in this constantly evolving field.
