Atopic dermatitis (AD) is a chronic, relapsing inflammatory skin disorder presenting significant burden to affected individuals and healthcare systems. Recent progress in understanding its immunopathogenesis has catalyzed the development of targeted biologic therapies, revolutionizing long-term management. This review provides a comprehensive, evidence-based overview of AD, with an emphasis on epidemiology, pathophysiology, clinical presentation, diagnostic strategies, and traditional as well as emerging therapeutic options. Special focus is placed on new biologics, their mechanisms, clinical trial outcomes, and integration into guideline-directed care for optimal patient-centered management.
Atopic dermatitis is a multifactorial, immune-mediated skin disease characterized by chronic pruritus and eczematous lesions, commonly beginning in childhood but frequently persisting or recurring in adulthood. Its substantial psychosocial and economic impact underscores the necessity for effective, durable management strategies. The advent of biologic therapies marks a paradigm shift in AD treatment, offering targeted interventions for patients with moderate-to-severe, refractory disease. An in-depth understanding of evolving therapeutic modalities is essential for healthcare professionals managing this complex condition.
AD affects approximately 15-20% of children and 2-10% of adults globally, with rising prevalence in urbanized regions. Disease onset commonly occurs within the first five years of life, though adult-onset cases are increasingly recognized. The chronicity, frequent flare-ups, and associated comorbidities—such as asthma, allergic rhinitis, and mental health disorders—contribute to a pronounced decrease in patient quality of life. Direct and indirect healthcare costs, absenteeism, and psychosocial consequences place AD among the most burdensome dermatologic diseases worldwide.
The pathogenesis of AD involves a complex interplay of genetic predisposition, epidermal barrier dysfunction, and immune dysregulation. Filaggrin gene mutations impair skin barrier integrity, facilitating allergen penetration and transepidermal water loss. Immune responses are skewed toward type 2 helper T cell (Th2)-driven inflammation, with elevated interleukin (IL)-4, IL-13, and IL-31 signaling. Chronic disease phases exhibit increased Th1, Th17, and Th22 activity, further perpetuating inflammation and barrier disruption. Microbial colonization—particularly Staphylococcus aureus—exacerbates inflammation and disease severity.
Genetic susceptibility is a major risk factor, with family history of atopy (asthma, allergic rhinitis, or AD) conferring increased risk. Environmental influences include urban living, pollution, climate extremes, and exposure to allergens or irritants. Early-life factors such as cesarean delivery, formula feeding, and antibiotic exposure may predispose to AD. Psychosocial stress, skin barrier impairment, and comorbid allergic disorders are additional contributors to disease development and persistence.
AD is characterized by pruritic, erythematous, and scaly lesions, with distribution varying by age. Infants often present with facial and extensor involvement, while flexural areas predominate in older children and adults. Chronic lesions may exhibit lichenification, fissuring, and excoriation. Recurrent bacterial, viral, and fungal skin infections are common due to barrier dysfunction and immune dysregulation. Severe pruritus frequently disrupts sleep and daily functioning, contributing to psychological morbidity.
AD diagnosis is clinical, based on characteristic morphology, distribution, and chronicity of lesions, supported by history of pruritus and atopy. Diagnostic criteria, such as Hanifin and Rajka or UK Working Party, assist in standardization. Laboratory testing is not routinely required but may include total IgE, eosinophil count, and specific allergen sensitization assessment in complex cases. Differential diagnosis includes contact dermatitis, psoriasis, seborrheic dermatitis, and immunodeficiency disorders. Skin biopsy is reserved for atypical or refractory presentations.
First-line management encompasses patient education, avoidance of triggers, and regular use of emollients to restore barrier function. Topical corticosteroids remain the mainstay for acute flares, with topical calcineurin inhibitors as steroid-sparing alternatives, particularly for sensitive areas. Antihistamines may alleviate pruritus, while phototherapy is considered for moderate-to-severe disease unresponsive to topicals. Systemic immunosuppressants—such as cyclosporine, methotrexate, and azathioprine—are reserved for severe, refractory cases due to adverse effect profiles.
Biologic therapies have transformed the AD treatment landscape, targeting specific cytokines implicated in disease pathogenesis. Dupilumab, a monoclonal antibody against the IL-4 receptor alpha subunit, inhibits both IL-4 and IL-13 signaling and is approved for moderate-to-severe AD in adults and children. Long-term studies confirm its sustained efficacy and favorable safety profile. Tralokinumab and lebrikizumab, both IL-13 inhibitors, have demonstrated significant improvement in clinical outcomes in phase III trials.
Janus kinase (JAK) inhibitors, including baricitinib and upadacitinib, offer oral, targeted immunomodulation with rapid onset of action. Clinical trials reveal substantial efficacy for moderate-to-severe AD, though long-term safety monitoring is essential due to risk of infections and thrombosis. Ongoing studies are evaluating novel agents targeting OX40, IL-31, and other immune pathways, promising further individualization of therapy.
Recent guidelines from the American Academy of Dermatology and European Dermatology Forum emphasize a stepwise, patient-centered approach. Topical therapies and skin care remain foundational, with escalation to phototherapy or systemic agents based on disease severity, impact on quality of life, and response to prior interventions. Biologics are recommended for moderate-to-severe AD unresponsive to standard therapies or where systemic immunosuppressants are contraindicated. Shared decision-making, regular monitoring, and multidisciplinary care are essential for optimal outcomes.
Advances in understanding the immunopathogenesis of atopic dermatitis have ushered in a new era of targeted biologic therapies, offering hope for improved long-term disease control and quality of life. Integration of emerging agents into evidence-based, guideline-directed care is reshaping the management paradigm for moderate-to-severe AD. Ongoing research, personalized treatment strategies, and multidisciplinary collaboration will be pivotal in addressing the complex needs of patients with this burdensome chronic disease.
