Unexplained weight loss (UWL), defined as a loss of 5% or more of total body weight within a six-month period, poses a diagnostic challenge in clinical practice. While it can be a harbinger of serious underlying pathology, it often remains unexplained despite thorough investigation.
UWL can be attributed to gastrointestinal disorders, malignancies, psychiatric conditions, endocrine disorders, or chronic infections. The initial evaluation should include a comprehensive history, physical examination, and basic laboratory tests. If these are inconclusive, further targeted investigations may be required based on the patient's history and clinical presentation.
Imaging studies, such as CT scans or MRIs, can be useful in detecting occult malignancies or other abnormalities. Specialized tests, such as endoscopy or colonoscopy, may be warranted in cases of suspected gastrointestinal disorders. However, these should be guided by the patient's symptoms and risk factors, to avoid unnecessary investigations.
Management depends on the underlying cause. Even if no cause is identified, it is important to monitor the patient closely, as the cause may become apparent over time. Nutritional support should be provided to all patients experiencing UWL, to prevent further weight loss and associated complications.
UWL is a complex clinical problem that requires a systematic approach for effective management. While it can be frustrating when no cause is found, clinicians should remain vigilant, as early detection of serious underlying conditions can significantly impact patient outcomes.
