This review synthesizes recent evidence and clinical guidelines to provide healthcare professionals with practical insights into epidemiology, pathophysiology, risk factors, clinical features, diagnosis, management strategies, and emerging therapies in coordinated survivorship care. Special emphasis is placed on the integration of primary and specialty care, late effects surveillance, and optimizing quality of life through evidence-based and guideline-driven interventions.
\nThe global increase in cancer survival rates, attributable to advances in early detection and multimodal therapies, has shifted the healthcare paradigm towards long-term survivorship care. Coordinated survivorship care aims to bridge the gap between active cancer treatment and routine health maintenance, encompassing surveillance for recurrence, management of late effects, psychosocial support, and health promotion. The complexity of survivorship trajectories underscores the necessity for collaborative care models, involving oncologists, primary care providers, nursing, rehabilitation, and mental health professionals, to ensure continuity and comprehensiveness of care for survivors.
\nGlobally, there are over 43 million cancer survivors, with numbers expected to rise due to aging populations and improved cancer treatments. In the United States alone, the survivor population is projected to surpass 22 million by 2030. As the survivor population grows, healthcare systems must prioritize integrated care models that address the full spectrum of survivor needs—medical, psychosocial, and socioeconomic. Ongoing research, innovation in care delivery, and policy advocacy will be essential to optimize outcomes and quality of life for cancer survivors in the years ahead.
