This article critically reviews the evolution, scientific basis, and clinical application of patient-centric methodologies in the management of gastrointestinal (GI) disorders. Drawing upon recent PubMed-indexed research and clinical guidelines, we explore epidemiologic trends, pathophysiological underpinnings, risk stratification, diagnostic advances, and therapeutic strategies that align with patient-centered care. Emphasis is placed on the integration of shared decision-making, personalized medicine, and multidisciplinary collaboration to optimize outcomes for diverse patient populations. The review aims to furnish healthcare professionals with actionable insights for implementing patient-centric models in gastroenterological practice.
The paradigm shift toward patient-centric care has transformed the landscape of modern medicine, particularly in specialties such as gastroenterology where chronicity, diagnostic complexity, and therapeutic diversity prevail. Patient-centricity moves beyond disease-focused models by prioritizing the preferences, values, and individual circumstances of patients. In gastroenterology, this approach is especially pertinent due to the high prevalence of chronic conditions such as inflammatory bowel disease (IBD), irritable bowel syndrome (IBS), and liver disorders, all of which demand nuanced, long-term management strategies. This review delineates the scientific rationale, clinical relevance, and practical implications of patient-centric care, referencing recent evidence and guideline-based recommendations.
Gastrointestinal diseases constitute a significant global health burden, with estimates indicating that digestive diseases account for millions of deaths and substantial morbidity worldwide. Chronic conditions like IBD affect approximately 0.3% of the global population, while functional GI disorders such as IBS impact up to 10–15%. Hepatobiliary diseases, including non-alcoholic fatty liver disease (NAFLD) and viral hepatitis, are emerging as major public health issues, particularly in the context of rising obesity and metabolic syndromes. The growing prevalence of GI disorders underscores the necessity for patient-centric strategies that address not only medical but also psychosocial, economic, and quality-of-life aspects.
The pathophysiology of GI diseases is multifactorial, involving complex interactions between genetic susceptibility, environmental exposures, immune dysregulation, and the gut microbiome. Patient-centric management in gastroenterology encompasses individualized therapeutic regimens that account for patient preferences, comorbidities, and lifestyle factors. This may involve selecting among biologic agents, immunomodulators, and small molecules in IBD based on efficacy, safety, and patient priorities. In functional GI disorders, holistic strategies integrating dietary modification, pharmacotherapy, psychological support, and behavioral interventions are advocated. Multidisciplinary care teams—including dietitians, psychologists, and nurse specialists—play a crucial role in delivering comprehensive, continuous care that aligns with patient goals.
Recent years have witnessed significant progress in the development of novel therapies that support patient-centric care. The advent of precision medicine—leveraging genomics, proteomics, and microbiome profiling—has enabled targeted interventions for subsets of patients with specific molecular signatures. For example, anti-integrin and anti-interleukin biologics offer tailored options for refractory IBD. Digital health tools, including mobile applications and remote monitoring platforms, facilitate ongoing patient engagement, symptom tracking, and telemedicine consultations, thereby enhancing patient autonomy and access to care.
Leading gastroenterology societies, including the American Gastroenterological Association (AGA) and European Crohn’s and Colitis Organisation (ECCO), advocate for patient-centric models as a standard of care. Guidelines emphasize the integration of shared decision-making, use of PROMs, and individualized risk assessment in clinical workflows. Recommendations highlight the importance of multidisciplinary collaboration and continuous patient education to achieve optimal outcomes. Implementation science underscores the need for system-level changes—such as electronic health record (EHR) integration and care pathway redesign—to operationalize patient-centric practices.
Patient-centric approaches represent a transformative advancement in gastroenterology, bridging the gap between scientific innovation and individualized care. By synthesizing pathophysiological insights, risk stratification, and multidisciplinary management, clinicians can deliver care that is not only evidence-based but also empathetic and responsive to patient needs. Ongoing research, technological innovation, and robust guideline development are essential to further refine and disseminate patient-centric models, ultimately improving outcomes and quality of life for patients with gastrointestinal diseases.
