Enteric fever, predominantly caused by Salmonella Typhi and Paratyphi, remains a significant health concern, particularly in developing regions. This article presents effective strategies for the diagnosis and management of enteric fever, aiming to guide healthcare professionals in delivering optimal patient care.
The gold standard for diagnosing enteric fever is blood culture, although its sensitivity varies. Rapid diagnostic tests (RDTs) like Widal and Tubex TF are often used in resource-limited settings, but their reliability can be compromised by prior exposure or vaccination. Molecular methods, such as PCR, show promise for their sensitivity and specificity, but require further validation.
Successful management of enteric fever heavily relies on effective antibiotic therapy. Fluoroquinolones were once the treatment of choice, but increasing resistance has led to a shift towards azithromycin and third-generation cephalosporins like ceftriaxone. Antibiotic stewardship is crucial to prevent further resistance development.
Vaccination is a key preventive measure. Currently, two vaccines, Vi-polysaccharide and Ty21a, are WHO prequalified. However, their protective efficacy is not 100%, and they are not recommended for children under 2 years. The newer conjugate vaccine (TCV), with broader age indication and higher efficacy, offers hope for better control of the disease.
Alongside clinical management, public health measures play a vital role in controlling enteric fever. These include improving sanitation and water quality, promoting hand hygiene, and educating communities about the disease transmission and prevention.
Effective diagnosis and management of enteric fever require a multifaceted approach, encompassing accurate diagnosis, appropriate antibiotic therapy, vaccination, and public health interventions. As healthcare professionals, our role extends beyond treating individual patients to contributing to wider efforts to control this persistent public health challenge.
