Abstract

Multimorbidity defined as the coexistence of two or more chronic conditions in an individual has emerged as the norm rather than the exception in geriatric medicine. With the global demographic shift towards an aging population, the complexity of managing multiple interrelated diseases has profound implications on clinical decision-making, resource allocation, and patient-centered outcomes. This review synthesizes current evidence on mapping multimorbidity complexity in advanced age, focusing on epidemiology, pathophysiology, risk factors, clinical features, diagnostic strategies, management paradigms, recent advances, and guideline recommendations. By elucidating the mechanisms underlying multimorbidity and highlighting practical mapping frameworks, we aim to enhance clinical care, foster interdisciplinary collaboration, and improve health trajectories for older adults.
