This review synthesizes epidemiological trends, pathophysiological mechanisms, risk profiles, diagnostic strategies, and therapeutic innovations in ENT, emphasizing evidence-based approaches and guideline-driven management. Particular focus is placed on emerging therapies and their translational potential in clinical practice, with insights into future directions for improved patient care.
The field of ENT encompasses a diverse array of disorders affecting the ear, nose, and throat, ranging from infectious and inflammatory conditions to neoplastic and congenital abnormalities. Over the past decade, the specialty has witnessed significant transformations driven by better understanding of disease mechanisms, diagnostic innovations, and therapeutic breakthroughs. This article aims to provide clinicians and healthcare professionals with a comprehensive, up-to-date overview of key developments in ENT, highlighting clinically relevant insights and the practical application of recent evidence to enhance patient outcomes.
ENT disorders constitute a significant global health burden, impacting all age groups and contributing to considerable morbidity and healthcare utilization. Chronic rhinosinusitis affects approximately 11% of adults worldwide, while otitis media remains a leading cause of pediatric consultations and hearing loss. Head and neck cancers, though less common, account for over 500,000 new cases annually, with rising incidence in some regions due to changing risk factors. The burden of obstructive sleep apnea and allergic rhinitis continues to escalate, further underscoring the need for effective preventive and therapeutic strategies in ENT practice.
Understanding disease mechanisms is pivotal for targeted interventions. Chronic rhinosinusitis, for instance, is increasingly recognized as a heterogeneous disorder driven by complex inflammatory pathways, including type 2 immune responses and epithelial barrier dysfunction. Similarly, sensorineural hearing loss often results from cochlear hair cell damage due to genetic, infectious, or ototoxic insults. Malignant transformation in head and neck cancers is linked to molecular alterations, such as HPV-driven oncogenesis in oropharyngeal tumors. Enhanced mechanistic insights are fueling the development of novel biomarkers and personalized therapies in the ENT realm.

Detailed history and systematic examination remain the cornerstone of assessment, guiding further diagnostic workup and management.
Diagnostic accuracy in ENT has improved with technological advancements. Nasal endoscopy, otoendoscopy, and flexible laryngoscopy facilitate direct visualization of mucosal and structural lesions. Radiological modalities, including CT and MRI, provide high-resolution anatomical detail, essential for preoperative planning and tumor staging. Audiometry and vestibular testing enable functional assessment in otologic disorders. Molecular diagnostics and immunohistochemistry are increasingly utilized for tumor profiling and infectious disease identification, paving the way for precision medicine approaches.
Optimal management of ENT disorders integrates pharmacological, surgical, and supportive modalities. Intranasal corticosteroids and saline irrigations remain first-line for chronic rhinosinusitis, with endoscopic sinus surgery reserved for refractory cases. Otologic infections are managed with antimicrobial therapy, while cochlear implantation offers auditory rehabilitation in severe hearing loss. Head and neck malignancies require multidisciplinary care involving surgery, radiotherapy, and systemic therapies. Airway management and CPAP therapy are vital in obstructive sleep apnea. Individualized treatment plans, informed by disease severity and patient factors, optimize outcomes and minimize adverse effects.
Innovations in ENT are rapidly translating into clinical practice. Artificial intelligence applications in imaging and diagnostics are poised to further improve accuracy and workflow efficiency.
Contemporary guidelines from societies such as the American Academy of Otolaryngology–Head and Neck Surgery and the European Position Paper on Rhinosinusitis and Nasal Polyps (EPOS) underscore the importance of evidence-based, multidisciplinary care. Recommendations emphasize stepwise management, shared decision-making, and the judicious use of antibiotics to mitigate resistance. Prophylactic vaccinations (e.g., influenza, pneumococcus, HPV) are advocated for at-risk populations. Regular updates reflect evolving evidence and best practices, supporting clinicians in delivering high-quality, patient-centered care.
Progressive insights in ENT are reshaping the landscape of clinical care, driven by advances in pathophysiological understanding, diagnostics, and therapeutics. Integration of emerging evidence and guideline recommendations enables personalized, effective interventions, ultimately improving patient outcomes. Ongoing research and innovation promise continued evolution in the field, with the potential to address unmet clinical needs and enhance the quality of life for individuals with ENT disorders.
