Unexplained weight loss (UWL), defined as a loss of more than 5% of body weight within six to twelve months without a clear cause, is a common yet challenging clinical scenario. It can be a harbinger of serious underlying diseases, making its evaluation crucial in clinical practice.
UWL can be attributed to a myriad of conditions. These include malignancies, gastrointestinal disorders, endocrine diseases, infections, and psychiatric illnesses. In some cases, medications or aging may also cause weight loss. However, in approximately 15-25% of cases, the cause remains elusive even after extensive investigation.
A systematic and comprehensive approach to evaluating UWL is essential. A detailed history and physical examination are the cornerstones of diagnosis, often providing clues to the underlying cause. Laboratory tests, imaging studies, and endoscopic procedures may be required based on clinical suspicion. A watchful waiting approach may be appropriate in some patients, given that the cause of UWL may become apparent over time.
While imaging is not routinely recommended in the evaluation of UWL, it may be beneficial in certain scenarios. For instance, CT scans can be useful in detecting occult malignancies or abdominal pathologies. However, the use of imaging should be guided by the patient's history and physical examination findings.
Management of UWL is primarily directed towards treating the underlying cause, if identified. In cases where no cause is found, symptomatic treatment and regular follow-up are essential. Nutritional support may be required in patients with significant weight loss.
In conclusion, evaluating UWL necessitates a comprehensive and systematic approach, with a focus on history taking, physical examination, and judicious use of investigations. While challenging, a meticulous approach can often yield the underlying cause, enabling targeted treatment and improving patient outcomes.
