Precision care for complex multimorbidity is an evolving paradigm that seeks to individualize medical management for patients with multiple chronic conditions. This review synthesizes recent evidence on epidemiology, pathophysiology, risk factors, clinical manifestations, and diagnostic strategies, while critically examining current and emerging approaches to personalized treatment. Emphasis is placed on the integration of genomics, digital health, and multidisciplinary care models, as well as guideline-based recommendations to optimize outcomes and minimize risks for this vulnerable population.
Complex multimorbidity, defined as the coexistence of three or more chronic conditions, represents a growing challenge in clinical medicine. Traditional disease-centric frameworks often fall short in addressing the intricate interplay between comorbidities, polypharmacy, and patient preferences. Precision care applies individualized strategies—incorporating genetic, phenotypic, and psychosocial data—to tailor interventions that improve quality of life, reduce complications, and enhance health system efficiency. This review aims to provide clinicians with a comprehensive understanding of precision care approaches for patients with complex multimorbidity and highlight practical implications for everyday practice.
The global prevalence of complex multimorbidity is increasing, driven by aging populations, advances in chronic disease management, and lifestyle factors. Epidemiological data suggest that up to 25% of adults, and over 65% of those aged 65 and above, live with three or more chronic conditions. The burden is disproportionately higher in socioeconomically disadvantaged groups, contributing to disparities in healthcare access, outcomes, and resource utilization. Multimorbid patients account for a significant proportion of hospital admissions, readmissions, and healthcare expenditures, underscoring the urgent need for precision-based, cost-effective solutions.
The pathophysiology of complex multimorbidity is multifactorial, involving shared genetic susceptibilities, chronic inflammation, immunosenescence, and dysregulated metabolic pathways. Interactions between comorbid conditions, such as diabetes and cardiovascular disease, can potentiate disease progression and complicate management. Systems biology approaches reveal that common molecular networks—such as the renin-angiotensin system, oxidative stress pathways, and cytokine signaling—may serve as potential therapeutic targets. The heterogeneity of multimorbidity phenotypes necessitates detailed mechanistic understanding to guide personalized interventions.
Risk factors for complex multimorbidity include advanced age, genetic predisposition, unhealthy lifestyle behaviors (sedentary behavior, poor diet, smoking), low socioeconomic status, psychosocial stress, and environmental exposures. Early-life adversity, mental health disorders, and social isolation further increase susceptibility. Recent cohort studies highlight the bidirectional relationship between physical and mental health, amplifying risk and complicating disease trajectories. Identification and modification of these risk factors are central to prevention and risk stratification in precision care models.
Patients with complex multimorbidity present with heterogeneous and often overlapping clinical features, including polypharmacy, functional impairment, cognitive decline, frailty, and reduced quality of life. Symptom burden is typically high, with frequent exacerbations and atypical presentations complicating assessment. Multimorbidity also increases the risk of adverse drug reactions, hospitalizations, and mortality. Patient-centered evaluation should encompass physical, psychological, and social dimensions to inform individualized care planning.
Diagnosing complex multimorbidity requires comprehensive assessment that extends beyond traditional organ-based approaches. Standardized tools, such as the Cumulative Illness Rating Scale and Charlson Comorbidity Index, aid in quantifying disease burden. Integration of electronic health records, biomarker profiling, and patient-reported outcomes enhances diagnostic accuracy and risk prediction. Multidisciplinary evaluation—including geriatric, psychiatric, and social work input—is essential for uncovering hidden comorbidities and tailoring care pathways.
Management of complex multimorbidity necessitates individualized, goal-oriented care plans that prioritize patient preferences and minimize treatment-related harm. Key strategies include medication reconciliation, deprescribing inappropriate therapies, and addressing modifiable risk factors. Multidisciplinary teams—comprising primary care, specialists, pharmacists, and allied health professionals—coordinate interventions to optimize symptom control, function, and adherence. Shared decision-making and advance care planning are critical for aligning treatment with patient values and expectations.
Recent advances in precision care leverage genomics, digital health technologies, and artificial intelligence to refine risk stratification and therapeutic targeting. Polygenic risk scores, pharmacogenomics, and omics-based profiling facilitate identification of high-risk phenotypes and prediction of drug response. Digital platforms enable real-time monitoring, remote consultations, and tailored interventions. Emerging therapies—including anti-inflammatory biologics, senolytics, and microbiome modulation—show promise in modifying disease trajectories for selected patients. Large-scale clinical trials are underway to validate these innovations in multimorbid populations.
Current guidelines endorse a person-centered, coordinated approach for managing complex multimorbidity. International bodies, such as the National Institute for Health and Care Excellence (NICE) and American Geriatrics Society, recommend comprehensive assessment, individualized goal setting, and regular review of care plans. Polypharmacy management, deprescribing, and incorporation of non-pharmacological interventions (e.g., rehabilitation, psychosocial support) are emphasized. Guidelines increasingly recognize the role of digital health and data-driven decision support in advancing precision care for multimorbid patients.
Precision care represents a transformative approach to the management of complex multimorbidity, integrating mechanistic insights, technological advances, and patient-centered strategies. By embracing individualized assessment, multidisciplinary collaboration, and evidence-based interventions, clinicians can improve outcomes and quality of life for this challenging patient population. Ongoing research and innovation are essential to further refine precision care models and ensure their equitable implementation in routine clinical practice.
