Chronic cough, defined as a cough persisting for more than eight weeks, presents a diagnostic and therapeutic challenge for clinicians. Despite its prevalence, the approach to chronic cough often lacks a systematic and comprehensive strategy, leading to delayed diagnosis and suboptimal management.
Initially, clinicians should differentiate between cough-variant asthma, gastroesophageal reflux disease, and upper airway cough syndrome, the three most common causes of chronic cough. A comprehensive medical history, physical examination, and judicious use of investigations like spirometry, chest radiographs, and upper gastrointestinal endoscopy can guide diagnosis. Additionally, the role of newer diagnostic modalities, such as impulse oscillometry and laryngeal sensory testing, is expanding.
Treatment should be etiology-specific and patient-centered. Empiric treatment trials can be considered when the diagnosis is uncertain. For cough-variant asthma, inhaled corticosteroids are the mainstay of treatment. Proton pump inhibitors are recommended for gastroesophageal reflux disease-associated cough, while antihistamines and nasal corticosteroids can be used for upper airway cough syndrome. Refractory cases may benefit from neuromodulatory agents like gabapentin and pregabalin.
Recent research has shed light on the phenomenon of cough hypersensitivity syndrome, a condition where cough receptors in the airway become hypersensitive, leading to chronic cough. This has opened new avenues for the development of novel therapeutic agents targeting the cough reflex pathway, such as P2X3 receptor antagonists.
In conclusion, a comprehensive and systematic approach to chronic cough is essential for accurate diagnosis and effective management. Emerging concepts and novel therapeutic agents promise to transform the landscape of chronic cough management in the future. Clinicians need to stay abreast of these developments to provide optimal care for patients with chronic cough.
