Unexplained weight loss (UWL), defined as a loss of more than 5% of body weight within a six to twelve month period without a known cause, can be a harbinger of serious underlying conditions. A comprehensive approach to diagnosing the cause is vital to provide appropriate management and improve patient outcomes.
The first step in the diagnostic approach is a thorough history and physical examination. It's essential to review dietary habits, medication use, psychosocial history, and symptoms suggestive of systemic diseases. The physical examination should focus on signs of malnutrition, wasting syndromes, and systemic diseases.
Initial diagnostic testing should be guided by clues from the history and physical examination. Basic tests include complete blood count, metabolic panel, liver function tests, thyroid function tests, and urinalysis. If these tests are unrevealing, consider targeted testing based on the patient's age, risk factors, and symptoms.
Imaging studies are not routinely recommended unless the history and physical examination suggest a specific disease. In the absence of specific indications, the indiscriminate use of imaging may lead to unnecessary procedures and increased patient anxiety.
In older adults, UWL may be a sign of malignancy, gastrointestinal disorders, or psychiatric conditions. In younger patients, consider endocrine disorders, malabsorption, or chronic infections. Always consider the possibility of eating disorders, especially in adolescents.
Unexplained weight loss can be a challenging diagnostic dilemma. A systematic and comprehensive approach, starting with a detailed history and physical examination, followed by selective diagnostic testing and judicious use of imaging, can effectively elucidate the underlying cause. Understanding the differential diagnosis based on the patient's age and risk factors can help guide the diagnostic approach and lead to timely and appropriate management.
