Fever is the body's adaptive response to pathogenic invasion, mediated by pyrogens that reset the hypothalamic thermostat. This understanding is fundamental to the evaluation of fever and aids in distinguishing it from hyperthermia, a detrimental condition resulting from failed thermoregulation.
Initial assessment should focus on history, physical examination, and relevant investigations for identifying potential sources of infection. Empirical antipyretics may be initiated while awaiting diagnostic results. However, the decision should consider the patient’s discomfort, potential benefits of fever in fighting infection, and risk of drug-related adverse effects.

Management strategies should be tailored to the specific etiology identified. In cases where the cause remains elusive, a symptom-oriented approach may be adopted. This includes regular monitoring, reassessment, and modification of the treatment plan based on patient response. Antipyretic therapy should be continued judiciously, balancing the benefits and potential harms.
Managing fever in clinical practice necessitates a nuanced understanding of its pathophysiology, a systematic approach to diagnosis, and a patient-specific management strategy. As healthcare professionals, staying abreast of the latest evidence and adopting a comprehensive approach can significantly enhance patient care outcomes in fever management.
