Atopic dermatitis (AD) is a chronic, relapsing inflammatory skin disorder that imposes substantial disease burden globally. Recent advances have revolutionized both its diagnosis and management through improved molecular understanding, enhanced diagnostic criteria, and the emergence of targeted therapies. This review synthesizes current evidence on epidemiology, pathophysiology, risk factors, clinical manifestation, diagnosis, and therapeutic strategies for AD, with a focus on recent breakthroughs and guideline-based recommendations. Key clinical implications for physicians are discussed, highlighting the translation of research findings into practice.
Atopic dermatitis is one of the most prevalent chronic inflammatory skin diseases worldwide, affecting both children and adults. Characterized by pruritus, eczematous lesions, and a relapsing-remitting course, AD not only impacts quality of life but also poses significant challenges in management due to its heterogeneous clinical presentation and complex pathogenesis. Recent scientific advances have led to a paradigm shift in the approach to diagnosis and treatment, providing clinicians with new tools and strategies to optimize patient outcomes.
AD affects approximately 15-20% of children and 1-3% of adults globally, with increasing incidence in industrialized countries. The disease imposes a considerable burden through direct medical costs, increased healthcare utilization, and indirect costs related to lost productivity and psychosocial impact. The chronicity of AD, coupled with frequent exacerbations, significantly diminishes patient and caregiver quality of life. Comorbidities such as asthma, allergic rhinitis, and mental health disorders increase the complexity of disease management and further exacerbate disease burden.
The molecular pathogenesis of AD is multifactorial, involving genetic predisposition, epidermal barrier dysfunction, immune dysregulation, and environmental factors. Mutations in the filaggrin gene (FLG) are strongly associated with impaired skin barrier function, leading to increased transepidermal water loss and heightened allergen penetration. Immune dysregulation is characterized by a predominance of Th2-type cytokines (IL-4, IL-13) in acute lesions, with chronic phases involving Th1 and Th22 responses. The interplay between barrier impairment and immune activation drives the hallmark features of pruritus, inflammation, and lichenification in AD.
Risk factors for AD include a family history of atopic disease, genetic mutations (notably FLG), early-life environmental exposures, urban living, and altered microbiome composition. Perinatal factors, such as maternal diet and antibiotic exposure, may modulate risk. Sensitization to food or inhalant allergens is common, especially in pediatric patients, and the presence of coexisting allergic diseases increases the likelihood and severity of AD.
AD classically presents with pruritic, eczematous lesions distributed on flexural surfaces in children and adults, and on the face and extensor surfaces in infants. Chronic scratching leads to lichenification and excoriations. Disease severity varies from mild to severe, with acute flares characterized by erythema, edema, and vesiculation. Secondary bacterial infections, particularly with Staphylococcus aureus, are common and can exacerbate disease activity. The chronicity of symptoms often leads to sleep disturbance and psychological distress.
Diagnosis of AD is primarily clinical, based on characteristic morphology, distribution, and chronicity of skin lesions, along with a supportive personal or family history of atopy. Several validated diagnostic criteria exist, including the Hanifin and Rajka, and the UK Working Party criteria. Recent advances include the use of non-invasive biomarkers, such as serum TARC (thymus and activation-regulated chemokine) and skin tape-stripping for molecular analysis, which may aid in early diagnosis and assessment of disease activity. Differential diagnosis includes contact dermatitis, psoriasis, seborrheic dermatitis, and cutaneous T-cell lymphoma, necessitating careful clinical evaluation.
Management of AD is multifaceted, aiming to restore skin barrier function, control inflammation, alleviate pruritus, and prevent flares. First-line therapy involves regular emollient use and avoidance of triggers. Topical corticosteroids remain the cornerstone for acute flares, while topical calcineurin inhibitors are valuable steroid-sparing agents, particularly for sensitive areas. Antihistamines have limited efficacy for pruritus, but may be beneficial in some patients. Systemic therapies, including cyclosporine, methotrexate, and azathioprine, are reserved for refractory cases. Adjunctive treatments such as phototherapy and antimicrobial agents are considered in selected cases, especially in the presence of infection or extensive disease.
The therapeutic landscape of AD has been transformed by the advent of targeted biologics and small molecule inhibitors. Dupilumab, an IL-4 receptor alpha antagonist, has demonstrated robust efficacy and safety in moderate-to-severe AD and is now established as a first-line systemic agent. Other biologics targeting IL-13 (tralokinumab, lebrikizumab) and JAK inhibitors (baricitinib, upadacitinib, abrocitinib) have shown promising results in clinical trials and are being integrated into practice. These agents offer rapid, sustained disease control with favorable safety profiles, particularly in patients with inadequate response to conventional therapies. Ongoing research into novel targets, such as OX40 and IL-31, holds further promise for individualized therapy.
Current guidelines from organizations such as the American Academy of Dermatology, European Dermatology Forum, and Japanese Dermatological Association emphasize a personalized, stepwise approach to AD management. Key recommendations include early and proactive use of emollients, individualized selection of topical and systemic agents based on disease severity and comorbidities, and regular assessment of disease control and quality of life. The integration of patient education, psychosocial support, and multidisciplinary care is critical for optimal outcomes. Guidelines increasingly endorse the use of biologics and JAK inhibitors in moderate-to-severe disease, reflecting the evolving therapeutic landscape.
Atopic dermatitis remains a challenging and burdensome disease, but recent scientific and clinical advances have markedly improved prospects for affected patients. Enhanced understanding of pathomechanisms, refined diagnostic tools, and the introduction of targeted therapies have revolutionized the management paradigm. Clinicians must integrate evidence-based strategies, guideline recommendations, and individualized care to achieve optimal disease control and improve patient quality of life. Ongoing research and emerging therapies continue to expand the armamentarium, offering hope for further progress in this dynamic field.
