Abstract

Chronic gastrointestinal (GI) disorders significantly impair patient's ability to enjoy food and participate in daily living, shaping both nutritional status and quality of life. This review synthesizes current evidence on the epidemiology, pathophysiology, risk factors, clinical features, diagnosis, management, and evolving therapeutic strategies for optimizing food enjoyment and daily function in those living with chronic GI conditions. Emphasis is placed on the multidimensional impact of these disorders, the mechanisms underlying altered food experience, and practical, evidence-based interventions to improve patient outcomes.

Introduction

Chronic gastrointestinal disorders, including irritable bowel syndrome (IBS), inflammatory bowel disease (IBD), functional dyspepsia, and gastroparesis, are prevalent conditions characterized by persistent or recurrent GI symptoms. Beyond physical discomfort, these disorders profoundly affect patient's relationship with food, often leading to anxiety, avoidance behaviors, and compromised daily functioning. Healthcare professionals must recognize and address these challenges using a multidisciplinary, guideline-driven approach that integrates recent advances in pathophysiology, diagnostics, and therapeutics.

Epidemiology / Disease Burden

Chronic GI disorders are estimated to affect over 40% of the global population, with functional GI diseases alone accounting for a substantial proportion of outpatient gastroenterology visits. IBS has a global prevalence of approximately 11%, while IBD affects up to 0.5% in Western populations. These conditions commonly manifest during young adulthood, though they can occur at any age. Assessment tools such as the Food Enjoyment Scale and GI-specific quality of life instruments are useful for quantifying the impact of symptoms on eating behavior and daily function. Identification of nutritional deficiencies and psychological comorbidities is essential for comprehensive care.

Treatment & Management

Management of chronic GI disorders aims to alleviate symptoms, restore food enjoyment, and optimize daily functioning. First-line interventions include dietary modification such as low FODMAP or exclusion diets and pharmacotherapy tailored to symptom profile (e.g., antispasmodics, prokinetics, laxatives, or anti-inflammatories in IBD). Behavioral interventions, including cognitive-behavioral therapy and gut-directed hypnotherapy, have demonstrated efficacy in reducing food-related anxiety and improving coping skills. Nutritional counseling and multidisciplinary care are critical for preventing malnutrition and disordered eating patterns.

Recent Advances / Emerging Therapies

Recent advances in the management of chronic GI disorders have focused on microbiome-targeted therapies, including probiotics, prebiotics, and fecal microbiota transplantation, with promising but variable results. Novel pharmacological agents, such as secretagogues and bile acid modulators, offer additional symptom control options. Digital health tools including mobile applications for symptom tracking and telemedicine-based dietary counseling are enhancing patient engagement and self-management. Mechanistic research into gut-brain signaling continues to inform the development of targeted therapies for food-related distress.

Guideline Recommendations

Contemporary guidelines from leading gastroenterological societies emphasize individualized, symptom-based management of chronic GI disorders. Recommendations include early dietary intervention, psychological support, and stepped pharmacotherapy. Regular assessment of nutritional status, food enjoyment, and quality of life is advocated. Shared decision-making and patient education are highlighted as essential for optimizing outcomes and minimizing the psychosocial impact of chronic GI conditions.
