Unani medicine, with its roots in Greco-Arabic tradition, provides a holistic framework for promoting longevity and functional independence in older adults. This review critically examines Unani lifestyle principles in the context of contemporary scientific findings, elucidating their mechanisms, clinical relevance, and integration with current geriatric guidelines. The article synthesizes epidemiological data, pathophysiological insights, risk stratification, clinical manifestations, diagnostic approaches, evidence-based management, recent advances, and guideline recommendations to offer a comprehensive perspective for healthcare professionals.

Introduction

Globally, populations are aging at an unprecedented rate, presenting unique challenges for healthcare systems. The quest for healthy aging encompasses not only the prevention of disease but also the preservation of physical, mental, and social well-being. Unani medicine, an ancient system rooted in the teachings of Hippocrates, Galen, and later Islamic scholars, emphasizes the balance of bodily humors (Akhlat) and lifestyle modifications (Asbab-e-Sitta Zarooriya) as the cornerstone of health promotion and disease prevention. This article explores how Unani lifestyle principles align with modern scientific paradigms of aging, offering clinicians actionable insights into comprehensive geriatric care.

Contemporary geriatrics supplements these approaches with validated instruments such as the Comprehensive Geriatric Assessment (CGA), frailty indices, and cognitive screening tools. Laboratory evaluation for inflammatory markers, endocrine function, and nutritional status further aids in risk stratification and early detection of age-associated pathologies.

Treatment & Management

Unani management of aging is built upon lifestyle modification, dietary regulation, pharmacotherapy, and regimental therapies (Ilaj bil Tadbeer). Individualized plans are formulated based on temperament and comorbidities. Dietary recommendations focus on easily digestible, nutrient-rich, and temperament-appropriate foods. Physical activity, tailored to the patient’s capacity, is encouraged to preserve muscle mass and mobility. Regimental therapies—such as massage (Dalak), cupping (Hijama), and hydrotherapy—are employed to enhance circulation and detoxification. Unani pharmacopeia offers botanicals like Asgandh (Withania somnifera), Saffron (Crocus sativus), and Senna (Cassia angustifolia) with antioxidant, anti-inflammatory, and rejuvenative properties, supported by emerging clinical data.

Recent Advances / Emerging Therapies

Recent research has intensified focus on the molecular underpinnings of Unani interventions. Phytoconstituents from Unani herbs exhibit modulation of sirtuins, telomerase activity, and inflammatory mediators, suggesting anti-aging potential at the cellular level. Clinical trials report improvement in physical performance, cognitive scores, and quality of life among older adults adhering to Unani regimens.
