The integration of digital platforms into survivorship offers opportunities for personalized follow-up, risk stratification, early detection of recurrence, and improved quality of life, while also presenting unique challenges requiring multidisciplinary and patient-centered approaches.
Oral cancer survivors face multifaceted challenges extending far beyond primary treatment, including the risk of recurrence, second primaries, functional impairments, psychosocial distress, and long-term comorbidities. The rapid evolution of digital health technologies has introduced innovative solutions for survivorship care, enabling continuous monitoring, timely intervention, and enhanced patient engagement. For clinicians, understanding the landscape of digital survivorship care is critical to optimizing outcomes and aligning with modern standards of patient-centric oncology.
Globally, oral cancer accounts for over 350,000 new cases and approximately 177,000 deaths annually, with higher prevalence in South Asia and certain European regions. Advances in early detection and treatment have led to increased survival rates, resulting in a growing population of survivors with ongoing healthcare needs. Survivorship care must address not only oncologic surveillance but also rehabilitation, prevention of secondary malignancies, and management of late effects, underscoring the importance of scalable and accessible approaches such as digital health platforms.
Oral squamous cell carcinoma, the predominant histological subtype, arises from cumulative genetic and epigenetic alterations in the oral mucosa, often initiated by carcinogenic exposures. The pathobiology involves dysregulation of cell cycle pathways, angiogenesis, immune evasion, and chronic inflammation. The cornerstone of survivorship care is multidisciplinary follow-up addressing oncologic surveillance, rehabilitation (speech, swallowing, dental), psychosocial support, and management of late toxicities. Digital interventions, including telemedicine consultations, electronic symptom diaries, and virtual rehabilitation modules, enhance continuity of care, adherence, and timely escalation when new issues arise.
