Fatigue, a common complaint in clinical practice, is often an ill-defined and elusive symptom. It can reflect a multitude of underlying conditions, both physical and psychological. A comprehensive approach is vital for accurate diagnosis and effective management.
Fatigue can be categorized as either acute or chronic. Acute fatigue is a normal response to physical exertion or stress and resolves with rest. Chronic fatigue, however, persists for more than six months and is not relieved by rest. It significantly impairs daily functioning and quality of life.
A thorough history and physical examination are the cornerstone in the evaluation of fatigue. The history should include the duration, onset, pattern, associated symptoms, and impact on daily activities. The physical examination aims to identify signs of systemic disease that may be causing fatigue.
Investigations should be guided by the history and physical examination. Routine tests include complete blood count, thyroid function tests, and markers of inflammation. Further investigations, such as imaging or specialized tests, may be needed based on clinical suspicion of specific conditions.
Management of fatigue is guided by the underlying cause. It may involve treating the primary disease, managing symptoms, improving sleep hygiene, and addressing psychosocial factors. In cases where no clear cause is identified, a multidisciplinary approach involving cognitive-behavioral therapy, graded exercise therapy, and pharmacological interventions may be beneficial.
Fatigue is a complex symptom that requires a comprehensive and individualized approach for effective management. Understanding the potential causes and adopting a systematic approach to diagnosis can aid in providing targeted treatment. Continuing research into the mechanisms of fatigue will further enhance our ability to manage this challenging symptom.
