A thorough history and physical examination are crucial in the initial evaluation. Key elements include the pattern of nerve involvement, symptom onset, progression, and associated systemic symptoms. A comprehensive neurological examination can help to determine the type of nerve fibers involved.
Basic laboratory tests such as complete blood count, serum glucose, renal and liver function tests, and vitamin B12 levels are essential. Further, nerve conduction studies and electromyography can help to localize the lesion, determine the severity, and differentiate axonal from demyelinating neuropathies.
Additional testing should be guided by clinical suspicion. Genetic testing is recommended for patients with a family history suggestive of hereditary neuropathy. Skin biopsy for intraepidermal nerve fiber density measurement can be considered in cases of small fiber neuropathy.
While not routinely used, imaging can prove invaluable in certain scenarios. Magnetic resonance imaging (MRI) is beneficial in suspected cases of nerve root involvement or if a mass lesion is suspected.
Treatment should be etiology-specific and symptomatic. Glycemic control is crucial in diabetic neuropathy, while immunomodulatory therapy is indicated for inflammatory neuropathies. Symptomatic treatment includes pain management, physiotherapy, and occupational therapy.
Peripheral neuropathy evaluation requires a methodical approach, combining clinical acumen with appropriate investigations. Understanding these nuances can facilitate accurate diagnosis and targeted treatment, ultimately improving patient outcomes.
