Chronic cough, defined as a cough persisting for over eight weeks, is a common and distressing symptom which often presents a diagnostic challenge to clinicians. Despite the availability of guidelines, the complexity of the condition often results in misdiagnosis and inappropriate management. This article aims to provide a comprehensive approach to diagnosing and managing chronic cough in clinical practice.
Diagnosis of chronic cough involves a systematic approach. The first step is to ascertain the duration and nature of the cough, and any associated symptoms. Key investigations include imaging studies, spirometry and bronchoprovocation tests, and upper airway endoscopy. Differential diagnosis should consider common causes such as gastroesophageal reflux disease (GERD), asthma, and upper airway cough syndrome (UACS), as well as less common etiologies such as lung cancer, bronchiectasis, and interstitial lung disease.
Management of chronic cough is primarily aimed at treating the underlying cause. Empiric therapy for the most common causes (GERD, asthma, UACS) can be initiated before the results of investigations are available. Patients with refractory cough may benefit from multidisciplinary management, involving a pulmonologist, gastroenterologist, and otolaryngologist. Non-pharmacological interventions, such as speech and language therapy, can also be beneficial.
Recent advancements in our understanding of the pathophysiology of chronic cough have led to the development of new therapeutic targets. The role of neuronal hypersensitivity in chronic cough has been recognized, leading to trials of neuromodulators such as gabapentin and pregabalin. Additionally, research into the role of inflammatory mediators has opened up avenues for novel therapies.
Chronic cough represents a complex clinical challenge that requires a systematic approach to diagnosis and management. By considering the common and less common causes, and by using a multidisciplinary approach to management, clinicians can improve outcomes for patients with this distressing symptom. Future research into the pathophysiology of chronic cough will likely provide novel therapeutic options.
