Fatigue, a common complaint among patients, often presents a diagnostic challenge to healthcare professionals. Its multifactorial etiology, ranging from lifestyle factors to serious medical conditions, necessitates a comprehensive clinical approach for accurate diagnosis and management.
Fatigue is a subjective feeling of tiredness, lack of energy, or exhaustion. It can be classified as either acute (short-term) or chronic (lasting more than six months). Acute fatigue is usually a normal response to physical and mental exertion, while chronic fatigue may indicate underlying medical problems.
A thorough patient history and physical examination are the cornerstones of diagnosing fatigue. The history should include questions about the onset, duration, severity, and impact of fatigue on daily activities. It should also explore potential contributing factors such as sleep habits, diet, exercise, stress, and medication use. The physical examination can help identify signs of systemic diseases that may cause fatigue.
Investigations should be guided by the clinical suspicion based on history and physical examination. Basic investigations may include complete blood count, thyroid function tests, and metabolic panel. Further tests, such as sleep studies or imaging, may be required based on the initial findings.
Management of fatigue depends on the underlying cause. Lifestyle modifications, including balanced diet, regular exercise, and adequate sleep, form the basis of treatment. Cognitive-behavioral therapy can be beneficial for patients with chronic fatigue syndrome. Pharmacological treatment may be required for conditions such as hypothyroidism or anemia.
Decoding fatigue requires a systematic approach that includes a thorough history, physical examination, and appropriate investigations. Understanding the potential causes and management strategies can help healthcare professionals provide effective care for patients with this common, yet often complex, symptom.
