Unexplained weight loss (UWL) remains a clinical conundrum, often signaling underlying systemic disorders. It is incumbent upon clinicians to adopt a comprehensive approach to this multifaceted issue.
UWL is typically defined as a loss of more than 5% of body weight over a period of 6-12 months without a clear etiology. It's a common symptom in various diseases, including malignancies, endocrine disorders, gastrointestinal diseases, and psychiatric conditions.
History-taking and physical examination are paramount in the initial evaluation. The former should focus on dietary habits, gastrointestinal symptoms, changes in taste or smell, and medication use. The latter should be comprehensive, with particular attention to signs of cachexia, lymphadenopathy, and abdominal masses.
Initial laboratory investigations should include complete blood count, metabolic panel, liver function tests, thyroid function tests, and inflammatory markers. If these are inconclusive, imaging studies, endoscopy, and biopsy may be warranted. It's crucial to consider the patient's age, associated symptoms, and risk factors while planning the diagnostic approach.
Management should be etiology-specific and may involve treating the underlying disease, nutritional support, or pharmacotherapy. In cases where no clear cause is found, a symptom-based approach, regular follow-up, and reassessment are essential.
UWL is a complex clinical entity that requires a comprehensive, patient-centered approach. A meticulous history, thorough physical examination, and judicious use of diagnostic tests are key to identifying the underlying cause. Management should be tailored to the specific etiology, with a focus on addressing the patient's symptoms and improving their quality of life.
