Vertigo is primarily a symptom of vestibular dysfunction. It occurs when there's a discrepancy in the inputs from the bilateral vestibular system, visual system, and proprioceptive feedback. It's crucial for clinicians to distinguish vertigo from dizziness, as the latter is non-specific and can be a symptom of various non-vestibular disorders.
A comprehensive history and physical examination are paramount in the evaluation of vertigo. Hallmark features such as episodic vertigo, associated auditory symptoms, and triggers can be indicative of peripheral causes like benign paroxysmal positional vertigo, Meniere’s disease, or vestibular neuritis. Conversely, continuous vertigo, associated neurological symptoms, and lack of triggers often suggest central causes such as cerebellar stroke or vestibular migraine.
Diagnostic tests like the Dix-Hallpike maneuver, head impulse test, and Romberg’s test can help confirm the diagnosis. Neuroimaging and audiometry may be necessary in cases where central causes are suspected. The HINTS (Head Impulse, Nystagmus, Test of Skew) protocol is a reliable tool for distinguishing stroke from peripheral vertigo in acute settings.
Treatment of vertigo is guided by the underlying cause. Peripheral vertigo can often be managed with vestibular rehabilitation and medications like antihistamines or benzodiazepines. Central vertigo requires a more aggressive approach, often involving antiplatelet or anticoagulant therapy, and occasionally surgery.
In conclusion, a thorough understanding of vertigo's pathophysiology, a systematic approach to clinical evaluation, and appropriate diagnostic testing are vital in managing this common yet complex symptom. By adopting such a comprehensive approach, clinicians can ensure accurate diagnosis, effective treatment, and improved patient outcomes in cases of vertigo.
