Chronic cough, defined as a cough persisting for more than eight weeks, is a common yet complex clinical presentation that often poses a diagnostic challenge. It significantly impairs a patient's quality of life and necessitates a comprehensive approach for effective management.
A thorough history and physical examination form the cornerstone of diagnosing chronic cough. An understanding of the patient's environment, occupational exposures, and lifestyle habits are crucial. The diagnostic algorithm usually involves ruling out common causes such as gastroesophageal reflux disease (GERD), upper airway cough syndrome (UACS), and asthma. If initial investigations fail to identify the cause, more advanced diagnostic modalities, including imaging studies and bronchoscopy, may be warranted.
Empirical therapy can be a valuable tool in the diagnostic process. For instance, a trial of proton pump inhibitors may help diagnose GERD-related cough, while an inhaled corticosteroid trial can indicate asthma as the underlying cause. However, the response to empirical therapy should not replace a thorough diagnostic evaluation.
Despite a comprehensive diagnostic approach, a significant proportion of patients remain with unexplained chronic cough. Such cases necessitate a careful re-evaluation and consideration of less common causes like non-acid reflux, somatic cough syndrome or psychogenic cough. Multidisciplinary collaboration involving pulmonologists, gastroenterologists, otolaryngologists, and behavioral therapists can often provide the best outcomes.
Chronic cough is a multifaceted clinical entity requiring a comprehensive and systematic approach for its effective management. A meticulous history, physical examination, and selective use of diagnostic tests form the foundation of this approach. Empirical therapy can aid in diagnosis but should not replace a thorough evaluation. For refractory cases, a multidisciplinary approach is often beneficial, highlighting the importance of collaborative care in managing this complex condition.
