Chronic cough, defined as a cough persisting for more than eight weeks, represents a significant challenge in clinical practice, often leading to diagnostic uncertainty and treatment dissatisfaction. It is imperative for healthcare professionals to adopt a comprehensive approach to effectively diagnose and manage this condition.
Diagnosis of chronic cough involves a meticulous patient history and physical examination, which can provide clues to potential underlying conditions. Further investigations, such as chest radiographs, spirometry, and high-resolution computed tomography, may be warranted in certain cases. Empiric treatment trials should be judiciously used and interpreted cautiously to avoid misdiagnosis.
Common conditions associated with chronic cough include gastroesophageal reflux disease (GERD), asthma, and upper airway cough syndrome (UACS). However, healthcare professionals should also consider less common etiologies such as non-asthmatic eosinophilic bronchitis, pulmonary fibrosis, and lung cancer, particularly in high-risk patients.
Treatment should be tailored to the identified underlying condition. Proton pump inhibitors may be beneficial in GERD, while inhaled corticosteroids are often effective in asthma and eosinophilic bronchitis. Antihistamines and decongestants can be useful in UACS. Patients with refractory chronic cough may benefit from multidisciplinary assessment and management, including speech and language therapy, physiotherapy, and psychological support.
Recent advances have led to the development of novel therapies targeting neuronal hypersensitivity, a key mechanism in chronic cough. These include P2X3 receptor antagonists and transient receptor potential (TRP) channel modulators. These emerging therapies hold promise for patients with refractory chronic cough, but further research is needed to establish their efficacy and safety.
Chronic cough is a complex and often challenging condition to manage. A comprehensive diagnostic approach, individualized treatment strategies, and the integration of emerging therapies can enhance patient outcomes and satisfaction. Continued research in this area is essential to further refine our understanding and management of chronic cough.
