Gestational diabetes mellitus (GDM) is a significant health concern that affects up to 14% of pregnancies, posing risks to both mother and fetus. Effective management is key to optimizing maternal and fetal outcomes.
Early detection and diagnosis of GDM are crucial for initiating proper management. Universal screening, employing oral glucose tolerance tests, is recommended between 24 and 28 weeks of gestation. For high-risk women, early screening during the first trimester is recommended.
Dietary intervention remains a cornerstone in GDM management. A balanced diet, low in simple sugars and high in complex carbohydrates and fiber, is recommended. Regular dietetic consultation can aid in individualizing nutritional plans and ensuring adherence.
Physical activity can improve glucose control in GDM. Moderate-intensity exercise, unless contraindicated, is recommended. Exercise plans should be individualized and adjusted according to maternal tolerance and fetal response.
If lifestyle modifications are insufficient, pharmacological treatment is indicated. Insulin therapy is the gold standard. However, oral hypoglycemic agents like metformin and glyburide are increasingly being used due to their convenience and cost-effectiveness.
Postpartum follow-up is essential as GDM increases the risk of type 2 diabetes. Glucose tolerance testing should be performed 6-12 weeks postpartum and subsequently every 1-3 years.
In conclusion, GDM management requires a multidisciplinary approach encompassing early detection, dietary modifications, physical activity, pharmacological treatment, and postpartum follow-up. Through comprehensive care, we can effectively optimize maternal and fetal outcomes in GDM.
