The resurgence of Tuberculosis (TB) globally, compounded by the emergence of drug-resistant strains, necessitates a thorough understanding of current diagnostic modalities. This article offers a comprehensive review of contemporary diagnostic approaches to TB in healthcare.
Molecular diagnostic tests, such as the Xpert MTB/RIF assay, have revolutionized TB diagnosis. These tests rapidly detect Mycobacterium tuberculosis complex (MTBC) and rifampicin resistance within hours. Furthermore, the World Health Organization (WHO) endorses these tests as initial diagnostic tools in individuals suspected of having MDR-TB or HIV-associated TB.
Despite technological advancements, sputum smear microscopy remains a cornerstone of TB diagnosis in resource-limited settings. It is simple, inexpensive, and can yield results promptly. However, its sensitivity is variable and significantly lower in HIV-infected patients, children, and extrapulmonary TB cases.
The Tuberculin Skin Test (TST) and Interferon-Gamma Release Assays (IGRAs) are used for diagnosing latent TB infection. While TST is cost-effective and widely used, it has limitations, including cross-reactivity with BCG vaccination and non-tuberculous mycobacteria. IGRAs, on the other hand, are more specific but costly.
Chest radiography is a valuable tool in TB diagnosis, providing indirect evidence of the disease. However, its interpretation requires expertise, and findings can be non-specific. It is often used as an adjunct to microbiological tests.
Accurate and timely diagnosis of TB is critical for appropriate patient management and disease control. While each diagnostic approach has its strengths and limitations, an integrated strategy using a combination of these tests can enhance diagnostic accuracy. Continuous research and innovation are vital to overcoming existing diagnostic challenges and curbing the global TB burden.
