Peptic ulcer disease (PUD) remains a prevalent gastrointestinal disorder that poses significant morbidity and mortality worldwide. The evolution of its management has been dramatic over the past few decades, thanks to the advent of novel diagnostic tools and therapeutic strategies.
Endoscopy and biopsy, the gold-standard diagnostic tools for PUD, have seen significant advancements. Enhanced imaging techniques like narrow band imaging and high-definition endoscopy have improved the detection and characterization of ulcers. Moreover, non-invasive tests like urea breath test and stool antigen test for Helicobacter pylori, the most common cause of PUD, have simplified the diagnostic process.
Proton pump inhibitors (PPIs) have long been the cornerstone of PUD treatment. Recent developments have led to the introduction of Vonoprazan, a potassium-competitive acid blocker, providing a more potent and long-lasting acid suppression. The management of H. pylori has also evolved with quadruple therapy replacing the traditional triple therapy, increasing the eradication rates.
For PUD complications such as bleeding, endoscopic therapy has emerged as a first-line treatment. Techniques like endoscopic clipping, thermal coagulation, and injection therapy have improved the control of ulcer bleeding. Moreover, endoscopic submucosal dissection (ESD) has been a game-changer for early gastric cancer associated with PUD.
The landscape of PUD management has significantly changed with advancements in diagnostic and therapeutic modalities. These improvements have led to better patient outcomes, including higher ulcer healing rates, increased H. pylori eradication, and improved control of complications. As healthcare professionals, it is crucial to stay updated with these advancements to provide optimal care to our patients.
