Abdominal pain, a common symptom encountered in clinical practice, presents a diagnostic challenge due to its diverse etiology. A systematic approach is necessary to efficiently identify the underlying cause and initiate appropriate management.
Begin with a thorough history and physical examination. The character, location, and timing of the pain, along with associated symptoms, can provide valuable clues. For example, colicky pain suggests a hollow viscus obstruction, while steady, severe pain may indicate inflammation or ischemia.
Based on the initial assessment, select appropriate laboratory and imaging studies. Commonly used tests include complete blood count, liver function tests, amylase, lipase, and urinalysis. Imaging studies like ultrasound, CT scan, or MRI may be necessary to further delineate the pathology.
Consider the patient's age, gender, and comorbidities. In women, gynecological causes must be ruled out. Elderly patients may present atypically. For example, acute cholecicystitis may present without fever or leukocytosis. Similarly, diabetic patients may have painless gallstone disease or silent myocardial ischemia presenting as abdominal pain.
Management should be etiology specific and may range from reassurance and symptomatic treatment in functional disorders to surgery in cases of acute appendicitis or perforated peptic ulcer. In unclear cases, a stepwise approach, starting with conservative management, is advisable.
Given the complexity of abdominal pain, a comprehensive, systematic approach is crucial. This involves a detailed history and physical examination, appropriate diagnostic testing, and consideration of special patient factors. Management should be tailored to the specific etiology. Regular follow-up is necessary to ensure symptom resolution and to reevaluate if the patient's condition does not improve.
