Definition and Prevalence

Hypertensive emergencies refer to severe elevations in blood pressure, exceeding 180/120 mm Hg, accompanied by evidence of impending or progressive target organ dysfunction. They are relatively rare, accounting for less than 1% of patients with hypertension, but can be life-threatening if not promptly managed.

Pathophysiology

The pathophysiology of hypertensive emergencies involves a sudden increase in systemic vascular resistance, leading to endothelial injury and the release of vasoconstrictive substances. This spirals into further blood pressure elevations, causing end-organ damage.

Clinical Manifestations

Patients with hypertensive emergencies often present with nonspecific symptoms such as headache, nausea, and shortness of breath. More severe manifestations include chest pain, altered mental status, seizures, and acute renal failure, among others.

Management

Management of hypertensive emergencies involves immediate blood pressure reduction, usually with intravenous antihypertensive agents. The goal is to reduce mean arterial pressure by no more than 25% within the first hour, then to 160/100-110 mm Hg over the next 2-6 hours to avoid precipitating ischemic complications.

Prognosis

With prompt recognition and treatment, the prognosis for hypertensive emergencies is generally favorable. However, delayed treatment can result in irreversible organ damage and high mortality rates.
