Peptic ulcer disease (PUD), characterized by the formation of ulcers in the stomach or duodenum, is a prevalent gastrointestinal disorder. The management of PUD has evolved significantly over the years, thanks to advancements in diagnostic techniques, pharmaceutical interventions, and understanding of the underlying pathophysiology.

Improved Diagnostic Techniques

Diagnosis of PUD has been revolutionized by non-invasive tests like the urea breath test and stool antigen tests for Helicobacter pylori, the most common cause of PUD. Endoscopy, while invasive, provides direct visualization of ulcers and opportunity for biopsy, making it invaluable in detecting gastric malignancies.

Pharmaceutical Interventions

Pharmacotherapy for PUD primarily aims at eradicating H. pylori and reducing gastric acid secretion. Proton pump inhibitors (PPIs) are the cornerstone of therapy, providing rapid symptom relief and ulcer healing. The advent of quadruple therapy, combining a PPI, bismuth, and two antibiotics, has improved H. pylori eradication rates. Recent advancements include the development of potassium-competitive acid blockers (P-CABs) which offer a faster onset of action and better acid suppression compared to PPIs.

Understanding the Pathophysiology

Increased understanding of PUD pathophysiology has led to the recognition of non-H. pylori, non-NSAID ulcers. These ulcers, often associated with stress, severe illness, or Zollinger-Ellison syndrome, require a different therapeutic approach. Additionally, the role of genetic predisposition in PUD has been acknowledged, paving the way for personalized medicine in PUD management.

The management of PUD has witnessed significant advancements in recent years. Improved diagnostic techniques have made detection of H. pylori more accurate. Pharmaceutical interventions, particularly PPIs and the emerging P-CABs, have revolutionized treatment. An increased understanding of the disease's pathophysiology has allowed for more targeted therapies. Despite these advancements, continuous research is necessary to further improve patient outcomes and reduce PUD-associated morbidity and mortality.
