Acute chest pain (ACP) is a common symptom that often presents a diagnostic challenge to clinicians due to its broad differential diagnosis. The potential causes range from benign conditions to life-threatening diseases such as acute coronary syndrome (ACS). Therefore, a comprehensive approach to diagnose and manage ACP is critical.
When a patient presents with ACP, the initial assessment should include a thorough history, physical examination, and electrocardiogram. Risk stratification is crucial in guiding management. Tools such as the HEART score can aid in identifying high-risk patients who may benefit from hospital admission and further cardiac evaluation.
Diagnostic testing is an integral part of the ACP evaluation. In addition to electrocardiogram, cardiac biomarkers, particularly troponins, have a key role in diagnosing ACS. Imaging studies such as chest X-ray or CT angiography may be necessary to rule out non-cardiac causes of chest pain.
Management should be tailored to the underlying cause of ACP. For suspected ACS, early reperfusion therapy is the standard of care. Non-cardiac causes of chest pain may require different treatment approaches, such as gastrointestinal therapy for gastroesophageal reflux disease or analgesia for musculoskeletal pain.
Follow-up care is essential to ensure resolution of symptoms and to implement preventive measures. Lifestyle modifications and pharmacotherapy may be indicated to reduce the risk of future cardiac events in patients with coronary artery disease.
ACP is a complex clinical issue that requires a comprehensive approach for effective diagnosis and management. Careful initial assessment, appropriate diagnostic testing, targeted management strategies, and diligent follow-up care are all vital components of this approach. By adopting such a comprehensive approach, clinicians can provide optimal care for patients presenting with ACP and potentially prevent severe cardiac events.
