Introduction

Enteric fever, encompassing both typhoid and paratyphoid fever, remains a significant global health concern. Despite advances in diagnostic methods and treatment regimens, the disease continues to pose challenges due to evolving antibiotic resistance and non-specific clinical presentation. This article outlines the latest approach to diagnosing and treating enteric fever.

Diagnostic Techniques

Diagnosis of enteric fever has traditionally relied on blood culture, which remains the gold standard. However, its sensitivity varies from 40-80%, necessitating the need for more accurate tests. Molecular diagnostic techniques, such as polymerase chain reaction (PCR), offer higher sensitivity and specificity. Novel rapid diagnostic tests (RDTs) based on antigen detection are also promising, with the Typhidot test showing considerable utility in resource-limited settings.

Antibiotic Treatment

The rise of multi-drug resistant (MDR) and extensively drug-resistant (XDR) strains of Salmonella Typhi has made treatment more complex. Fluoroquinolones, once the drug of choice, are now less effective due to resistance. Current guidelines recommend using third-generation cephalosporins like ceftriaxone or cefixime, or azithromycin as first-line treatment. In cases of XDR strains, carbapenems or azithromycin are recommended.

Vaccination

Vaccination remains a key preventive measure. The World Health Organization recommends the typhoid conjugate vaccine (TCV) for children over six months in endemic areas. The older Vi polysaccharide vaccine is recommended for travellers to endemic areas, but does not provide long-term protection like TCV.

Conclusion

Enteric fever continues to be a global health challenge. Accurate diagnosis is pivotal and can be enhanced with molecular techniques and RDTs. Treatment is complicated by increasing antibiotic resistance, necessitating vigilant antibiotic stewardship. Vaccination remains a crucial component of prevention. As healthcare professionals, staying abreast of these evolving strategies is essential in managing this persistent disease.
