Chronic cough, defined as a cough persisting for more than eight weeks, presents a diagnostic challenge to clinicians. Despite its high prevalence, the etiology often remains elusive, leading to a significant impact on patient's quality of life. This article aims to provide a comprehensive approach to diagnosing and managing chronic cough in clinical practice.
The initial step in diagnosing chronic cough involves a thorough history and physical examination. This should focus on identifying common causes such as gastroesophageal reflux disease (GERD), upper airway cough syndrome (UACS), and asthma. If these common causes are ruled out, further investigation may involve imaging studies, pulmonary function tests, and bronchoscopy. It is also crucial to consider less common causes such as obstructive sleep apnea, bronchiectasis, and cardiac cough.
Management of chronic cough should be tailored according to the underlying cause. For instance, proton pump inhibitors may be beneficial in GERD-associated cough, while inhaled corticosteroids can be used for asthma-related cough. In cases where the cause remains unidentified, symptomatic treatment using cough suppressants may be considered. However, it is important to monitor the patient's response to treatment and adjust the therapeutic strategy as needed.
Given the complex nature of chronic cough, a multidisciplinary approach involving pulmonologists, gastroenterologists, and otolaryngologists can be beneficial. This collaborative approach can facilitate a comprehensive evaluation, ensuring no potential cause is overlooked.

While common causes should be the initial focus, clinicians should be prepared to delve deeper when these are ruled out. A multidisciplinary approach can be invaluable in this context, providing a holistic view of the patient's condition and facilitating effective management.
