Gestational diabetes mellitus (GDM) is a common complication in pregnancy, affecting approximately 7% of all pregnancies. It is associated with an increased risk of adverse maternal and neonatal outcomes. This article provides a comprehensive guide for healthcare professionals on managing GDM to optimize outcomes.
Early detection of GDM is crucial for optimal management. Universal screening using a two-step approach, starting with a 50g glucose challenge test followed by a 100g oral glucose tolerance test for those who screen positive, is recommended. The diagnosis is made based on the Carpenter and Coustan criteria or the National Diabetes Data Group criteria.
Management of GDM involves lifestyle modifications, glucose monitoring, and pharmacotherapy if necessary. Dietary modifications and physical activity are first-line interventions. Self-monitoring of blood glucose is essential, and targets should be individualized. If glycemic targets are not met within one to two weeks, pharmacotherapy should be initiated. Insulin is the preferred medication, but oral hypoglycemic agents may be used under certain circumstances.
Postpartum care is an integral part of GDM management. Women with GDM have a sevenfold increased risk of developing type 2 diabetes. Therefore, they should be screened for persistent diabetes at 6-12 weeks postpartum and should undergo lifelong screening for the development of diabetes or prediabetes.
Managing GDM effectively requires a comprehensive approach that includes early screening and diagnosis, lifestyle modifications, glucose monitoring, pharmacotherapy when necessary, and diligent postpartum care. By adopting these strategies, healthcare professionals can significantly improve the outcomes for both mother and baby, and reduce the risk of future metabolic diseases.
