Thyroid nodules are common, with palpable nodules affecting 4-7% of the population. However, the prevalence increases to 19-68% when detected by ultrasound. While most are benign, 5-15% are malignant, necessitating a thorough evaluation.
Initial evaluation includes a detailed history and physical examination. Risk factors such as radiation exposure, family history of thyroid cancer, and rapid nodule growth should be considered. The physical examination should include palpation of the thyroid gland and assessment of regional lymph nodes.
High-resolution ultrasound is the primary imaging modality for thyroid nodules. It can provide information about nodule size, location, echogenicity, and other features that may suggest malignancy. Fine-needle aspiration biopsy is the most accurate method for distinguishing benign from malignant nodules.
Management depends on the clinical and cytological findings. Benign nodules may be monitored, while suspicious nodules may require surgery. Follow-up includes regular clinical and ultrasound evaluations to monitor nodule growth and potential changes.
Thyroid nodule evaluation is a complex process that requires a systematic approach. Understanding the prevalence, risk factors, diagnostic modalities, and management strategies can aid in accurate diagnosis and optimal patient management. As healthcare professionals, it is essential to stay updated with the latest guidelines and advancements in this field.
