Gestational diabetes mellitus (GDM) is a common medical complication of pregnancy, affecting up to 14% of pregnancies worldwide. The condition is associated with numerous short-term and long-term maternal and fetal complications. Hence, healthcare professionals need to be equipped with the knowledge and skills to effectively manage GDM to optimize outcomes for both mother and baby.
Early detection of GDM is crucial for effective management. Universal screening, using either the one-step or two-step approach, is recommended between 24 and 28 weeks of gestation. However, high-risk women should be screened at the first prenatal visit. Diagnosis is based on elevated plasma glucose levels during an oral glucose tolerance test.
Management of GDM primarily involves lifestyle modifications, including a balanced diet and regular physical activity. Medical nutrition therapy is the cornerstone of treatment, aiming to provide adequate nutrition while maintaining euglycemia. If lifestyle modifications are insufficient, insulin therapy may be required. Oral hypoglycemic agents, such as metformin or glyburide, are alternatives when insulin is contraindicated or unacceptable to the patient.
Regular monitoring of maternal glycemic control and fetal growth is vital. Self-monitoring of blood glucose and regular antenatal visits are recommended. Postpartum follow-up is essential to screen for type 2 diabetes and provide lifestyle counseling to reduce long-term risk.
Effective management of GDM requires an interdisciplinary approach, involving obstetricians, endocrinologists, dietitians, and diabetes educators. A team-based approach ensures comprehensive care and enhances patient adherence to management plans.
Optimizing outcomes in GDM requires timely diagnosis, effective management strategies, regular monitoring, and an interdisciplinary approach. By staying abreast of the latest evidence-based practices, healthcare professionals can significantly improve the health outcomes of women with GDM and their offspring.
