Introduction

Chronic Obstructive Pulmonary Disease (COPD) is a progressive lung disease that is increasingly prevalent worldwide. It is characterized by persistent respiratory symptoms and airflow limitation due to airway and/or alveolar abnormalities. This article aims to provide a comprehensive overview of the current diagnostic and treatment strategies for COPD.

Diagnosing COPD

Early diagnosis of COPD is critical to manage the disease effectively. The diagnosis is based on a combination of history, physical examination, and confirmatory spirometry. The presence of a post-bronchodilator FEV1/FVC < 0.70 confirms the presence of persistent airflow limitation and thus COPD in patients with appropriate symptoms and significant smoking history.

Treatment Strategies

The primary goal of COPD treatment is to reduce symptoms, reduce the frequency and severity of exacerbations, and improve health status and exercise tolerance. Treatment strategies include smoking cessation, pharmacotherapy, pulmonary rehabilitation, and oxygen therapy. Inhaled corticosteroids, long-acting beta2-agonists, and long-acting muscarinic antagonists are the mainstay of pharmacotherapy. Pulmonary rehabilitation is a comprehensive intervention based on a thorough patient assessment followed by patient-tailored therapies.

Oxygen Therapy and Surgical Interventions

Oxygen therapy is recommended for patients with severe resting hypoxemia. Surgical interventions like lung volume reduction surgery, bullectomy, and lung transplantation are considered for patients with advanced COPD who have not responded to other treatments.
