Chronic cough, defined as a cough lasting more than eight weeks, is a common yet challenging condition encountered in primary care and respiratory clinics. It often presents a diagnostic puzzle, with patients undergoing numerous investigations to identify its etiology. This article offers a comprehensive approach to diagnosing and treating chronic cough, aiming to enhance patient care and clinical outcomes.
Chronic cough may result from a wide range of conditions. The most common causes include gastroesophageal reflux disease (GERD), postnasal drip syndrome, and asthma. However, less common causes such as lung cancer, sarcoidosis, and foreign body aspiration should also be considered. A thorough history and physical examination, along with appropriate diagnostic testing, are crucial in pinpointing the cause.
Initial evaluation should focus on identifying common causes. If these are ruled out, consider less common causes and referral to a respiratory specialist. Chest X-ray, spirometry, and upper airway endoscopy can be useful initial investigations. In refractory cases, high-resolution computed tomography (HRCT) and bronchoscopy may be warranted.
Treatment should be targeted towards the underlying cause. For GERD-related cough, proton pump inhibitors may be beneficial. Inhaled corticosteroids are often effective for asthma-related cough, while antihistamines and decongestants can alleviate postnasal drip syndrome. In cases where no cause is found, empirical treatment with cough suppressants or neuromodulators may be considered.
Chronic cough is a complex condition requiring a systematic and comprehensive approach for effective management. Identifying the underlying cause is paramount and often requires a combination of clinical assessment and diagnostic testing. Tailored treatment strategies can significantly improve patient outcomes, reducing morbidity and improving quality of life. Continued research and clinical discussion are needed to further refine our approach to this challenging clinical entity.
