Parkinson's disease (PD) is a chronic, progressive neurodegenerative disorder affecting approximately 1% of individuals aged 60 and above. The disease is characterized by the loss of dopaminergic neurons in the substantia nigra pars compacta, leading to motor and non-motor symptoms.
The hallmark pathological feature of PD is the presence of Lewy bodies, abnormal aggregates of the protein alpha-synuclein, in the remaining dopaminergic neurons. The exact mechanism of neuronal death is still under investigation, but it is believed that oxidative stress, mitochondrial dysfunction, and protein misfolding all play a role. The loss of dopamine in the basal ganglia results in an imbalance of excitatory and inhibitory signals, leading to the characteristic motor symptoms of PD.
PD presents with a combination of motor symptoms (tremor, bradykinesia, rigidity, and postural instability) and non-motor symptoms (depression, cognitive impairment, and autonomic dysfunction). Diagnosis is primarily clinical, based on the presence of motor symptoms and response to dopaminergic medications. Neuroimaging can be useful in uncertain cases, but is not routinely recommended.
Management of PD is multidimensional, involving pharmacological and non-pharmacological approaches. Pharmacological treatment is centered around augmenting dopamine levels in the brain, primarily through the use of levodopa. Non-pharmacological strategies include physical therapy, occupational therapy, and speech therapy. Deep brain stimulation is an option for patients with advanced disease and severe motor fluctuations.
Understanding the pathophysiology and management of PD is crucial for healthcare professionals to provide optimal care for these patients. While there is currently no cure for PD, ongoing research promises the potential for new therapeutic strategies in the future.
