Interstitial lung diseases (ILDs) represent a heterogeneous group of more than 200 chronic lung disorders. They are characterized by inflammation and scarring of the lung interstitium, leading to progressive dyspnea and impaired gas exchange. This article provides a comprehensive overview of ILDs, focusing on etiology, clinical features, diagnostic approaches, and treatment strategies.
ILDs can be classified based on their etiology into idiopathic, occupational, drug-induced, and those associated with systemic diseases. The most common form is idiopathic pulmonary fibrosis (IPF), the cause of which remains unknown. Occupational and drug-induced ILDs are caused by exposure to harmful substances or certain medications respectively, while systemic diseases like rheumatoid arthritis or scleroderma may also involve the lung interstitium.
Patients with ILDs typically present with chronic, progressive dyspnea and non-productive cough. Physical examination may reveal bibasilar inspiratory crackles and finger clubbing. Advanced stages may manifest with signs of right heart failure due to pulmonary hypertension.
High-resolution computed tomography (HRCT) is the cornerstone of ILD diagnosis, showing characteristic patterns like honeycombing in IPF. Pulmonary function tests reveal a restrictive pattern with reduced diffusion capacity. In certain cases, a lung biopsy may be necessary to confirm the diagnosis.
Treatment of ILDs is largely dependent on the underlying cause. For idiopathic forms, antifibrotic agents like pirfenidone and nintedanib have shown efficacy in slowing disease progression. Corticosteroids and immunosuppressants are used in ILDs associated with systemic diseases. Lung transplantation remains the only curative option for advanced disease.
ILDs are complex disorders requiring a comprehensive diagnostic approach and individualized management strategies. Continued research into their pathogenesis and treatment is crucial for improving patient outcomes. It is important for healthcare professionals to stay updated on the latest advancements in this field.
